Trouble getting the Blackmagic Intensity Pro to work

I recently purchased and installed an Intensity Pro (not the 4k) capture card on my Windows 7 computer, but have not been able to get it "see" the video via the HDMI input. The input video is 720p, and I've tried several apps (including Desktop Video) to see if was working. The apps all recognize the card, and even seem to be receiving the audio, but no image is displayed.

I believe this to probably be a driver issue, but have no idea which driver package I should install or if it matters. Can anyone tell me which driver package should be installed or if there's another explanation for my problem?
