Intensity Card error

Hi There, We have installed the intensity card in three different pc's,with the same result.When you click on the Capture command,on the software,the application freezes and says:"BlackMagic Media express has encountered a problem and needs to close".We installed the card,then loaded and updated the software.I also updated the pc's graphics,bios and chipset drivers with no luck.Please help.Is the card faulty maybe?
