I think you already suspect hardware failure?
Your only option is to test that card on another computer.

Well, if you suspect a hardware failure it is absolutely possible that the input failed but not the output as they are different parts of the board.

That card has a 1 year warranty. If it's within that time (or really close, I don't nitpick days), we can exchange it for you.

Yeah, If the board is that old I would highly doubt the component failure would be corrected with a reflow. Even so, we don't really make that sort of information public.

I wish I could be more helpful on this. :(
