Composite to SDI using Analog to SDI Mini Converter

Maybe this will help someone else trying to figure out to connect and configure their Analog to SDI MIni Converter, for use with a composite input. We couldn't find it on the Blackmagic web site or here in the forums when setting this up for our church. This what we discovered through trial, error and some guesses.
