Intensity Pro capture with Premiere 5.5

Just received my Intensity Pro card and I'm trying to capture with Premiere CS 5.5. When I start capturing, it works fine up until the point where I pan the camera. Is this a settings issue, a hard drive issue (not using SSDs) or something else?

Here are my settings:

Media Express:
Project Video Format: 720p 59.94
Capture File Format: AVI Motion JPEG

Premiere Pro:
Preset: Blackmagic Design --> HD720 --> Motion-JPEG --> 59.94p

Anything else needed?
