To narrow down the problem, I would test the Intensity Shuttle using the Blackmagic Media Express application. Once you confirm that the video signal is passing, then move on to OBS. Also keep in mind that the Intensity Shuttle does not support full HD (1080p/60), so make sure that the camera is not set to output that signal.
