Issues Getting Started With Decklink 4k Extreme 12G

I am having some troubles getting started and could use any assistance available.


Issue is the following

I have 3 systems i have been attempting to get this card (and now its replacement) to work in

First System is a:
OS: Ubuntu 17.10 artful (x68-64)
MB: MSI-990FXA-GD80
CPU: AMD 8150P
Ram: 16GB
GPU: amd 390
PSU: ULT-H600W

Second System is a:
OS: Windows 7 Pro 64-bit
MB: gigabyte ax370-gaming k3
CPU: AMD Ryzen 1800x
Ram: 32GB
GPU: AMD Vega 64
PSU: Raidmax Vampire RX-1000GH

Third System is a:
OS: Ubuntu 17.10 artful (x86-64)
(while i would love to give out the spec i honestly cant find them)
HP Z210 MN: SQ034UP
