Intensity Pro PCIe OSX Black video only

Hi

i have had an Intensity Pro for many years now
we used it once when first bought however it has not been used in the years since (just sitting in my machine)
today i have been trying to get it running with no luck

i must have rebuilt my Mac since purchase as the drives were not installed.

i installed Blackmagic_Desktop_Video_Macintosh_10.10 (did all the restarts and Intensity FW updates it requested)
however the Intensity did not show up in Adobe PP as a capture device
also Media Express would not open as I am running OSX 10.10.5

so i uninstalled Desktop video 10.10 and installed desktop video 10.1.1
this time Media express would open however the video input was black
and the Intensity did not show up in Adobe PP as a capture device

i have also now tried
Desktop Video 10.2
Desktop Video 10.2.2
Desktop Video 10.3.4

same result on all of them.

any one have any ideas on what to try next?
