Media Express not showing preview

I have the following setup:
BMD Card: Intensity Pro 4K
OS: Win 7
Desktop Video: 10.9.10 & 10.11.2
Problem: The Media Express application is not showing the preview of the video but rather a portion of the desktop.
It appears to capture video but there is no preview.
I've tried with both of the mentioned drivers but it doesn't fix anything.
Thoughts?
