Tiffen OFX Plugin issues with R12

The Tiffen DFX OFX plugin is not working properly when rendering the clip with the effects applied. It is rendering with thick black bars appearing sporadically during the render. This is occurring with both the main render and the smart render to any format.

Tiffen DFX OFX v4.0 (64bit)

I'm having the same problem with FilmConvert. Random glitching during renders.
