DeckLink Quad 2 not found on CentOS

my DeckLink Quad 2 is not listed as a PCI device on CentOS
after power cycling a few times it shows up as expected

05:00.0 Multimedia video controller: Blackmagic Design DeckLink Quad 2
for me, this looks like an HW issue but I am not sure maybe someone here can help me with this?
