The Desktop Video software available from the support page for your particular OS should not give you any troubles with the HD Extreme.

Hi Tony, I have been having an issue with my DeckLink HD Extreme 2 under OS X High Sierra.
Every Desktop Video iteration since Blackmagic_Desktop_Video_Macintosh_10.9.7 causes the card to "disappear" from the Audio panel. Only way to get it back is to reboot the system. Initially I thought it might be Premiere Pro causing the issue but I just did a clean install of the OS X hoping to fix the issue but it persists. Works fine with Adobe Creative Suite under 10.9.7.

Any thoughts on the issue and compatibility with OS X?

As these cards are legacy products, testing with them has not been done in quite some time. That said, we would expect them to still continue working but if you notice that a particular version of DTV video works correctly for your card, you may want to remain on that build specifically.
