Premiere CC 2019 Mojave Ultrastudio not seen

I have a BM Ultrastudio Express which runs perfectly on premiere cc 2018 / High Sierra. Temporarily upgraded to Mojave, Driver 11.1 and premiere cc 2019. The device now is not an option in Playback settings... any ideas?

I have an UltraStudio HD Mini that works fine with Blackmagic Express but Premiere CC 2019 does not see it as a capture device. I have Desktop Video 11.1 installed, with Mojave 10.14.5.

What gives?

Update:

This is resolved. I had to re-install the Desktop Video package *AFTER* installing Adobe Premiere. Then I rebooted the Mac. When I opened the same project in Premiere, I went to File > Capture > Capture Settings > [Settings] button, and now I see Blackmagic Capture as an option. First tests went perfect, captured 1080p60 for several minutes on a 2017 MBP with no dropped frames.

Perhaps Blackmagic can make this a little clearer in the instructions.
