ATEM 1ME issue with Tally?

We have a ATEM 1ME and have the tally interface. We had a professionally made cable to breakout to the new Sony HSC 300R CCU. I have 2 questions:

1. When we select the camera in PGM, we get Green Tally lights and not RED. Some times we get red, but goes away. Any idea? We connect via SDI

2. Sometimes the panel buttons go RED and the tally indicators flash on both the remotes and view finder on the camera. The panel still works, just looks like it is freaking out.

Seems like the breakoutcable is badly done or done for an old port standard.
I got the HSC300 CCU's recently to and our breakoutcables i made myself work fine.
