Full rig case / duffel?

Does anybody have a good camera bag solution for carrying a fully-rigged camera? I like to be ready to shoot as fast as possible so showing up rigged and ready is essential. I have looked everywhere. B&H sells more "camera purses" than actual camera bags. Pretty much all camera backpacks are copies of the same backpack. Regular duffel bags don't offer support or protection. Pelican cases are great but they don't have storage for small items (screws, adapters, etc.) A small case within the case could be a solution.

Hoping to find a rectangular duffel bag with rigid outer and padding plus lots of zippered storage. Ideally one that could double as a backpack for the more remote shoots.
