If you haven't tried BM's UltraNR, it's a selection in the spatial NR settings. Click "Analyze," it puts a tiny box somewhere in the frame and sets the two spatial NR parameters, chroma and luma. Then drag a corner of the box to expand it to the size for the area of interest, then drag the box over to that area and cover it with the box. As you move the box, see the settings change. You can stop at any point, and manually override the settings, and finally a global mix slider to blend in exactly the amount of NR action you want. The UltraNR does an especially good job extracting the noise without smudging the fine details. Zoom in 500-800% to examine the effect closely and remove just the noise and not the details.
The downside to UltraNR is it's really, really processor intensive, but it does such an exacting job it's worth it.
