Is there are a way of getting Resolve to list all the transitions used in a project (and preferably on which tracks and which times?
List of transitions used?

Yes, there is a way, simply under Edit Index, you will see in list view all transitions you have added including duration, and if you select it, it will take your play head at the start of that transition.

The Edit Index can be very useful for things like this.
