This may or may not be the issue, but it's worth checking... A common error when using transitions is a failure to provide long enough handles on the clips being transitioned. A "handle" is the extra footage before the "In" point of a clip or after the "Out" point of a clip.
In a transition, you need frames in the incoming clip before the actual clip boundary and you need frames in the outgoing clip after the clip boundary - this is because the transition uses images from both clips through the entire duration of the transition. For example, in a cross dissolve the incoming clip's images are being faded in to start blending with the outgoing clip even before the playhead reaches the "In" point of the incoming clip.
If you've cut a clip too close to the beginning or the end, or if you haven't included enough extra frames to cover the length of the transition when creating your Fusion composition, the transition may not have the frames it needs to work.
If adding the transition causes a failure, this is something to check.
