b) it could be related to just not a complete key. Use the gamma/gain function in viewer in fusion to make sure there is no garbage left from the keying process.
c) it could be related to out of gamut colors or related to floating point workflow, negative values. To clip them or bring them back into range, use brightness contrast node and use clip black and white checkbox and or (Pre-Divide / Post-Multiply). You can also use gamut node or color transform node to deal with out of gamut colors if there are any.
d) it could be just a bug. To make sure its also not some kind of cache issue, clear the cache and re-render the frame again, just to make sure. You can easily do that by right mouse clicking on the indicator how much cache is used in the lower right hand corner.
That is all that comes to mind at the moment.
