Did you perhaps have a second monitor connected at some point, or rearrange your screens? This commonly happens when the Add Tool dialog pops up on that second screen.
If you're using Windows (I suspect so; I haven't yet heard a Mac user ask about this one), there's a trick to getting that offscreen window back to your main screen.
Activate the Add Tool window with Ctrl+Space. Alt+Space to activate the Window manager menu (still invisible; you'll have to just trust that it's open). Type m to activate Move mode, then tap one of the arrow keys. This will cause your mouse pointer to vanish—it's now on the other screen, and the Add Tool window is "stuck" to it. Move the mouse, and Windows will relocate both the pointer and the window to your main screen. Left-click to drop the window, and it should appear on that monitor in the future.
