BSOD: blackmagicio.sys Crash Triggered by Driver Verifier

Hello everyone,

I have a Z790 motherboard with a Decklink Quad 2 card in an x8 slot, and I’m consistently getting a “0x0000007E: SYSTEM_THREAD_EXCEPTION_NOT_HANDLED” BSOD. Using Driver Verifier specifically on “blackmagicio.sys” (the only driver I selected) causes the system to crash on the first reboot and forces me to restore to a previous point. I’ve tested every other driver on this PC with Verifier without any issues—only blackmagicio.sys triggers the crash.

I’ve also tried multiple versions of Blackmagic Desktop Video (12.8.1, 12.9, up to 14.4), but the crash still occurs once Verifier is enabled. Each time I switch versions, I’m prompted to update the firmware multiple times, which seems excessive. I removed my NVIDIA GPU and switched to Intel HD Graphics, but no difference.

Does anyone have any insight into what might be causing this? Could the Decklink card itself be faulty? Thank you!

Error code on BSOD is 0x0000000A This BSOD means that Microsoft Windows or a kernel-mode driver accessed paged memory at DISPATCH_LEVEL or above. BSOD error code 0x0000000A may also show "IRQL_NOT_LESS_OR_EQUAL" on the same blue screen.

The hardware costs significant amounts of money and on a well-kept, up to date Windows 11 Pro PC, this shouldn't happen...and its extremely disappointing to see the above user posted in January and not a single person from BlackMagic has had any input on the matter!

@shafiro - Did you find a resolution??

I'm also using a Decklink Quad 2. Was actually in process of using DaVinci Resolve when the PC crashed. How has not a single BMD employee responded to this thread?!

I have encountered this issue before, and the following steps resolved it for me:

1- First, remove the card and uninstall the driver.
2- Update the motherboard BIOS.
3- Reinstall the card in the computer.
4- Install the 12.8.1 driver, but if it prompts for a restart during or at the end of the installation, cancel it and make sure to shut down instead (this shutdown is the most important trick to solve the issue.).
5- Upon powering it back on, if it prompts to update the firmware, definitely approve it.
6- In Device Manager, disable all Blackmagic AVStream devices.

Thanks for the reply. I've removed the drivers, removed the device, shutdown and restarted without the decklink card... Then, powered off, reinstalled and followed all the steps listed above.

While I was prompted to restart the PC after the 12.8.1 drivers were installed, i said no and shutdown. When i was receiving the BSOD errors, it would (often) only happen after my PC was powered on for a long while...so I don't know if this has fixed anything, but will repost and revisit here, if there's more issues.

I have a ASUS TRX40XE Gaming motherboard - and as it turns out, I had an outdated BIOS, by a couple of years. That has also been remedied. I genuinely hope this is the first and last I hear of this problem.
