DeckLink Mini Monitor 4K

Hi,
We've had this card for about a year, no issues and performs as expected. However, two days ago, it is now showing two wide vertical bars with black in between them. One green and one blue.
-Desktop Video Setup is running Driver 11.6
-The SDI out of DeckLink Mini Monitor 4K card is fed to a Flanders Scientific model XM551U UHD OLED monitor.
Has anyone seen something like this? Is it maybe re seat the card issue?
Any insight is appreciated.

Not sure of the specific problem, but the fix for us is to to toggle the LUT setting on the monitor.
