DeckLink HD Extreme video output issues on Premiere CS6

I'm having issues with DeckLink driver 9.6.9 and 9.6.8 with Adobe Premiere CS6 (6.0.2). When I play video out over SDI of the DeckLink HD Extreme, the video flickers and there is noise on the audio channels. I am using a sequence preset with YUV 10bit uncompressed.

If I downgrade to DeckLink driver 9.5.3, the video plays out without distortion.

I am running Mac OS X 10.7.5.
