Decklink 4k Extreme extend desktop not working

OSX 10.8.2
BMD Desktop Video 9.7
Decklink 4k extend desktop not working. Desktop gets extended, but nothing is displayed on SDI.

Is the desktop extending out at 4K? Do you have it connected to a supported 4K display? Did you gather the windows in the sys pref/display dialog, and then check the resolution and refresh rate feeding out?
