Extreme audio distortion on streaming and recording

We use an ATEM Mini Extreme ISO for live-streaming worship every Sunday, and we record each service directly to an SSD as a backup in case the streaming doesn't work. We've been doing this since June 2021.

Today both the streamed audio and the recorded audio have extreme distortion, to the point of being unintelligible. The sound monitored from the headphones jack at the time was fine. The streaming is via ethernet and the SSD is connected to the USB-C port. At first I thought it must have been a streaming issue, but the fact that the recording has the same problem indicates it's internal to the ATEM.

The ATEM software version is 9.01.

Any thoughts on what might cause this or suggestions on how to troubleshoot it?

UPDATE: I just checked the .WAV audio file on the SSD and it is clear, so at least I can now reconstruct the worship service to upload. Still very puzzling as to why the audio in the .mp4 video file is distorted.
