Is there a way to use an iPhone to connect the audio of a call, connect this into an ATEM and have the viewers of a live stream hear the call and the host?

Would have nothing to do with the atem since it's just audio. You would need some way of getting audio in and out. Interfaces like this are widely available.

Than the output of your phone would go to the audio mixer and a mix minus (PGM minus the phones input) would go to your phone from the audio mixer.

Any core audio compliant usb audio interface can be attached to an IOS device through the camera adaptor. You can then use a real microphone as the mic and route the audio to send the correct feed and to avoid feedback etc. Even if you do this it can take care of a lot of issues in doing this. You will still need an audio mixer to do a good job but using an audio interface can give you more flexibility and lets the host use a normal mic.
