How do I set up decklink4k USING HDMI as input from PTZ CAME

Hi Ed here-a non geek tech volunteer at our church and am trying to use DECKLINK 4K CARD hdmi port as input from a PTZ OPTICS CAMERA. How do I set it up? I use OSB SOFTWARE to broadcast our single sunday service. When I choose BLACKMAGIC WDM CAPTURE my monitor display has all lines going across the camera shot. HELP!
