Did you shut down your computer after the firmware update? The installer told me to do that at the end of the install on Mac. Not sure if that applies to Windows too...

Card may be corrupted during the firmware uodate. This happened to my Decklink two weeks ago. Send the card to BMD UK and they fixed it quickly. Give them a call!
